- Once the steps above are completed, our electronic health record (Charm EHR) will create your chart and auto-generate an invitation for you to access our patient portal.
Step 4.
- Follow the steps to setup your password, providing any additional information the system may request. Before continuing please validate accuracy of all information and submit any errors that may need correction.
Step 5.
- Within the questionnaire section of your chart there will be consent forms to be signed and returned.
Step 6.

Once these are received, labs will be ordered and a
requisition will auto-generate to your email address on file. Print and bring the requisition to
any Lab Corp location.

- Once results finalize, they will be sent to our attention. We will then reach out to schedule your consultation, during which we will discuss lab results and medication details. Any questions or concerns you may have will be addressed to your satisfaction.
Step 8.
- Once program fees are paid, medication and supplies will be prescribed. The pharmacy will process your prescription and send it overnight by refrigerated delivery to your address on file.
